Managing type 2 diabetes alongside other conditions like pulmonary sarcoidosis can be challenging. Here are some steps you can take based on the knowledge base:

1. Lifestyle Changes:
- Diet: Focus on a lower-calorie diet, healthy eating, and mindful carbohydrate intake to maintain healthy blood glucose levels.
- Exercise: Increase physical activity to improve insulin sensitivity and lower blood glucose levels.

2. Medication Management:
- Consult Your Doctor: If lifestyle changes and current medications are not enough, discuss with your doctor about adding or adjusting medications. Options include:
- Sulfonylureas: Increase insulin release (e.g., glyburide, glimepiride).
- Thiazolidinediones (TZDs): Decrease glucose release by the liver and improve insulin effectiveness (e.g., pioglitazone, rosiglitazone).
- Insulin Therapy: Continue adjusting your insulin dosage with your doctor's guidance.

3. Monitor and Adjust:
- Blood Glucose Monitoring: Regularly check your blood glucose levels to ensure they are within the target range.
- Medication Timing: If side effects are an issue, talk to your doctor about adjusting the timing of your medications.

4. Support and Reminders:
- Set Reminders: Use alarms or apps to remind you to take your medications.
- Family Support: Ask family members for help in managing your treatment plan.

5. Vaccinations:
- Ensure you are up to date with recommended vaccinations, such as the annual flu vaccine, pneumococcal vaccines, and others based on your age and health conditions.

Always consult with your healthcare provider before making any changes to your treatment plan.

Thank-you for the prompt response and information. T2 is a roller coaster, sarcoidosis is a cheap thrill, put them together and WOW! Keeps life exciting! 😎

Probably not a great deal except maybe talking to your Doctor about switching your Basil to a Mixed Insulin if you are still seeing some highs

The Prednisone will absolutely jack your blood sugar - it needs the sugar to do it's job so "shouts really load" until the Liver opens the tap and lets the sugar flow - nothing you can do about that unless you can treat your CV condition with a "non-steriod"

However, that being the more immediately important condition to treat, the Diabetes will simply have to be managed as best as possible. A mixed insulin may be part of the solution
